Office Information: About Your Visit
Insurance
Most insurance plans are accepted at Ambulatory Foot & Ankle Center, P.C. Patients will need to bring insurance card(s) and photo ID when coming in for a visit. We will need authorization from patient's primary care physician if required by the insurance company. Copays are due at the time of the appointment. For convenience, we accept Visa, MasterCard, American Express and Discover cards, as well as cash and personal checks.Appointments
We always welcome new patients. You should arrive 15 minutes prior to your scheduled appointment time, or you can download the forms from our website, complete and bring in at appointment time. Also, bring in insurance card(s) and driver's license or photo ID. A medical history form is available on-line to complete and bring in, or please be prepared to complete in the office. We will need a list of your current medications including dosage and strength. Write down any issues that you would like to discuss with the doctor. When coming in for a procedure, please remember to remove any nail polish and wear comfortable shoes.
Ambulatory Foot & Ankle Center, P.C. reserves the right to charge a fee for appointments not kept or cancelled within 24 hours.
Walk-in patients are welcome based on appointment availability. If you are in need of a prescription refill, please allow 48 hours for us to honor your request.
Fees and Payments
For billing inquiries, please contact our office at 757-825-5783, and one of our administrative staff members will gladly assist you. Insurance claims are processed outside of our office through a contracted billing service. If your inquiry cannot be resolved within our office, you will be provided the telephone number of the billing service. Patients are responsible for non-covered services that are provided by our office.
